As a result, each person has unique needs. Advanced Case Management works with the client, their family, and healthcare professionals to help assess, plan for, and implement ways to allow for their greatest degree of health, safety, independence, and quality of life.
In order to best serve each clients’ needs, Advanced Case Management designs a customized care plan composed of a spectrum of services. Care often requires a multi-disciplinary approach that encompasses many aspects of life such as healthcare, activities of daily living, transportation, finances, and emotional well-being. Many of our strategies focus on health and mental health, advocacy, home safety, and advance planning.
Advanced Case Management enlists a seven-step approach to ensure the success of our clients.

The first step at Advanced Case Management is the Intake. During an Intake, we get to know the potential client and compile basic information in order to asses eligibility.
The Assessment step allows us to get to know the person further. In this stage, we identify the status of well-being and the magnitude of the problem(s) in order to determine the nature and level of the interventions.
Once the potential client has been assessed and determined eligible for our services, an interview is then scheduled. This initial meeting establishes rapport and helps our team begin to provide structure, support, and accountability for the client.
Case Planning is a crucial stage in our process. The Case Plan is an important tool in supporting the client and establishing a “contract” between the client and the Care Team. During this time, the client and Care Team write measurable and outcome-oriented goals, identify resources, and determine intended outcomes of our case management services.
At Advanced Case Management, we believe that face-to-face meetings is crucial to success. Regular appointments with the client and the client’s family enable the Care Team to monitor progress according to each specific goal. Meeting in-person and reviewing goals on a regular basis allow the Care Team make adjustments to the Case Plan as needed for success.
At Advanced Case Management, we care about our current and future clients. In order to provide the best care, we conduct evaluations after each case to measure the outcome and determine if results were achieved.
Conducting internal and external follow-ups allow Advanced Case Management to determine if the Case Plan was followed, if services were received, and if goals were achieved.
Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality outcomes.
The underlying premise of case management is based in the fact that when an individual reaches the optimum level of wellness and functional capability, everyone benefits: the individuals being served and their support systems.
Case management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. Case management services are best offered in a climate that allows direct communication between the case manager, the client, and appropriate service personnel, in order to optimize the outcome for all concerned.
Certification determines that the case manager possesses the education, skills and experience required to render appropriate services based on sound principles of practice.
